Depending on the program, students may need to complete:

- Immunization documentation
- Tuberculosis screening
- Drug screening
- Healthcare worker background check
- Healthcare Provider CPR certification

- Program-specific eligibility documents
- Signed Enrollment Agreement and required acknowledgments
The online application currently requests government ID, a high school diploma or GED, education history, emergency-contact information, and additional records when applicable.
